Carbocaine stings less than any other anesthetic, so it's always the first anesthetic injected at any site. Other longer-acting anesthetics combined with carbocaine give consistently good anesthesia.

Mepivicaine without vasoconstrictor is used many times for patients who have a hypersensitivity to epinephrine. This 3% anesthetic works just fine, and lasts long enough for most operative procedures. Mepivicaine does not last as long as Lidocaine with vaso, and is not as profound as Septocaine 4% with epinephrine, but is a good alternative for patients who cannot or do not want vasoconstrictor. For many years, I have used a combination technique with Lidocaine 2% 1:100,000 epi and Mepivicaine 3% plain for patients who can take epinephrine. This affords profound anesthesia with mandibular block injections as well as infiltration injections on the upper arch. There is a synergistic effect of more profound anesthesia that just one type of anesthetic alone.
