Since the advent of 4% Articaine, 1:100 épi, those days are just about over.
A 1CC BUCCAL INFILTRATION is the typical dosage and location. This will anesthetize every mandibular tooth for restorative procedures. The incredible maxillary penetration eliminates the need for the palatal injection in most cases, and when necessary, is practically painless when you allow enough time for the buccal infiltration to penetrate.
For root planing, 3 slowly spaced buccal infiltrations will anesthetize the entire quadrant. By waiting for its amazing lateral penetration, those 2nd and 3rd pinches are painless! Think of all the patient and DDS anxiety restoring the 6-11 area with crowns. The patient s ânes this experience can drastically affect the outcome in many ways! Can you picture 2 buccal infiltrations between 6-7, and 10-11 will anesthetize the entire sextant! If you need an additional 8-9 pinch, that will not be felt at all. I already mentioned how painless those palatal pinches are if you wait.
The pinch is the most feared part of any procedure, and your patients never forget this experience. They become really impressed when you show your concern as you tell them about this technology. They appreciate the 1/2 dosage, the idea that only the needles tip has to penetrate them, (and that they don't have to become sword swallowers), and that it's fairly long duration means no additional pinches as that procedure nears its ending time. As for technique, an initial pinch of a few drops in super slow motion turns the rest if the dose into a gentle pushing sensation within 30 seconds. This bbrand of Articaine is the same as the name brand, in every way. It has replaced 90% on my anesthesia needs in my generall practice.
